	please could anyone set me in the right direction with this question, how and where to access info and support that can form your practice when assessing individual needs and preferences, and when contributing to the development, implementation and review of care plans. many thanks

	Hi butterfly, Dave is right - a question at last! How and where can you find out about how to assess individuals' needs and how to contribute to developing, implementing and reviewing care plans? You could ask for support from your colleagues or manager. Ask for training in your appraisal. Look at the minimum standards for your area of care. Ask for feedback from the individual, key people etc. Just a few ideas. Hope that helps Aunty Sue

	thank you very much for your help, much appreciated it makes sense once u explain it like that, and yes it has took me long enough for first question . I do have another if u might be able to help 328 what are the role of relationships and support networks in the assesment of individuals needs care planning, implementation and the review many thanks

	Hi butterfly, Think about the importance of your own family and friends. They are there to offer advice, support you in your decisions and make you feel loved and cared for and so on. An individual may not be able to express their wants and needs or answer the questions required in care planning. Family and friends who know them well can support them in this. They may also be involved in providing some of the care required. Hope that helps Aunty Sue The comments weren't aimed at you but the forum in general!
